  • Apprenticeship Educational Expenses Summary
    • Must include tuition, book fees, and lab fees only
  • Invoices from educational institutions
  • Proof of payment: copies of canceled checks or credit card statements
  • Certificate of Good Standing or authority to transact business in Illinois
  • Copy of USDOL-Approved Standards of Apprenticeship
    • Include all appendices
  • ETA-671 Forms for each apprentice
    • Usually located in the appendices of the Standards of Apprenticeship
  • Completed ITR-1 Form
  • Map of Underserved Areas
