- Must include tuition, book fees, and lab fees only
Invoices from educational institutions
Proof of payment: copies of canceled checks or credit card statements
Certificate of Good Standing or authority to transact business in Illinois
- Get from ilsos.gov
Copy of USDOL-Approved Standards of Apprenticeship
- Include all appendices
ETA-671 Forms for each apprentice
- Usually located in the appendices of the Standards of Apprenticeship
Completed ITR-1 Form
